The US Army has released a comprehensive investigative report detailing the systemic failures that caused a catastrophic 2025 midair collision between a military Black Hawk helicopter and an American Airlines regional jet near Washington.

The disaster, which occurred over the icy Potomac River and killed all 67 individuals aboard both aircraft, was attributed to outdated altimeter technology, severe communication lapses with federal aviation authorities, and critical airspace design flaws. The newly published military assessment aligns with earlier findings from the National Transportation Safety Board, underscoring how deeply entrenched bureaucratic friction and unheeded safety warnings ultimately led to the deadliest US aviation disaster since 2001.

US Army Report: What Did The Military Investigation Uncover?

The military assessment concluded that technological discrepancies placed the helicopter directly into the descent path of the commercial passenger jet as it approached Reagan National Airport. Investigators determined that the Black Hawk was flying between 24 and 30 metres higher than its barometric altimeter indicated.
