Three days after Tennessee’s failed attempt to execute death row inmate Christa Pike left her critically ill in a Nashville hospital, the state’s prison system is facing a leadership shake-up. Tennessee Department of Correction Commissioner Frank Strada will resign as Governor Bill Lee orders an independent investigation into what went wrong during the September 30 lethal injection.
Pike survived two doses of pentobarbital and remains under medical care, while her lawyers seek to preserve evidence from the execution and challenge her death sentence.

Frank Strada, commissioner of the Tennessee Department of Correction, will resign following the failed execution of Christa Pike, Governor Bill Lee announced on Saturday, October 3. Strada offered to step down as the state begins an independent review of the execution attempt, and Lee accepted his resignation.
The leadership change comes just three days after Pike survived two doses of pentobarbital during an execution attempt at the Riverbend Maximum Security Institution in Nashville. Instead of dying in the execution chamber, Pike was taken to a hospital, where her lawyers say she remains unconscious and on a ventilator.
Strada said his decision to leave was “in the best interests” of the state and said the investigation into the failed execution was “entirely appropriate and necessary”.
Lee said Strada had served the state with integrity.
“Commissioner Strada has served with integrity, and I appreciate his willingness to put the interests of Tennesseans first during this difficult moment,” Lee said.
Who is Frank Strada?
Strada became Tennessee’s corrections commissioner in January 2023 after serving as deputy director of the Arizona Department of Corrections. Governor Lee appointed him to lead the Tennessee Department of Correction after a career spanning decades in corrections and law enforcement.
His resignation now comes as questions grow over his oversight of executions in Tennessee and his previous corrections work in Arizona.
The Guardian reported that Strada had been involved in overseeing several executions that later drew criticism from attorneys and experts, including three in Arizona before he moved to Tennessee. The report also noted that his tenure in Tennessee included the failed Carruthers execution and the Pike case.
Those issues are now expected to receive additional attention as the independent review examines the Pike execution.
